Overview

Antibiotics are grouped based on the UK-AWaRe classification. These are:

Access
Access antibiotics are with a narrow spectrum of activity, fewer side effects, lower costs, and importantly lower resistance potential. These are first or second choice antibiotics recommended for empiric treatment of the most common infections and should be widely available. That is to say, these antibiotics should offer the best therapeutic value while minimizing the potential for resistance.

Watch
Watch antibiotics have a higher resistance potential and are broader-spectrum antibiotics. These are first or second choice antibiotics indicated for a limited number of infective syndromes. As these antibiotics are considered to have a higher potential for selection of AMR, their use should be carefully monitored.

Reserve
Reserve antibiotics are “last resort” antibiotics, including new antibiotics. These are used for highly selected patients (life-threatening infections due to multi-drug-resistant bacteria) and are closely monitored and prioritized as targets of stewardship programmes to ensure continued effectiveness.

Note 1: The chart shows a three-month rolling average, so the data shown for one month is the average across that month and the previous two months. Note 2: Interpreting national antibiotic resistance surveillance trends spanning European Committee on Antimicrobial Susceptibility Testing (EUCAST) clinical breakpoint transition periods requires additional consideration to avoid misinterpretation of data. Note 3: Specimen setting was defined using fields available in SGSS AMR and Trust data available from the 2023/24 Estates Return Information Collection (ERIC). Acute Trust specimens are those recorded by the laboratory as being taken in an acute trust setting or those with site codes that matched an ‘Acute’ trust type in ERIC. GP specimens are those recorded by the laboratory as being taken at a GP surgery or those not matching a trust type in ERIC; in addition, from previous validation work, codes of non-GP sites were also excluded from the GP specimens grouping. Presentation by setting may be impacted by local laboratory data reporting, whereby laboratories that do not differentiate between specimen settings may inaccurately represent the split in Acute Trust versus GP specimen setting for that area.
